As consideration for my child being allowed to participate in the programs and activities offered at General Daniel Chappie James Flight Academy Summer Camp (GDCJFA), I agree to release, waive, discharge and covenant not to sue GDCJFA along with its officers, directors, board members, supervisors, agents, servants, volunteers and/or employees (together referred to as “staff”) from any and all liabilities, claims, demands, or causes of action, including but not limited to negligence and negligent acts, that may arise from or be related to any and all physical and/or emotional loss, damage, and/or injury, including but not limited to death, that may be sustained by my child or my child’s personal property while my child is participating in the camp’s programs and/or activities or while my child is on the premises on which the camp is located and/or while my child is participating in any event set up, planned and/or coordinated by GDCJFA and/or its staff. As used in this section and Section II (Medical Information Agreement), “programs and/or activities” include, but are not limited to, those in connection with transportation (including, but not limited to, by motor vehicle, bicycle, boat and/or airplane), food, lodging, educational services and instruction, entertainment, games, arts & crafts, photographs, videos and physical and/or mental tasks. __________________________________________________________________________________________________________________________________________________________________________________________________________________________________________________ I, as an ordinary and knowledgeable person and of my own free will, understand this document to express a clear and understandable intention by myself and GDCJFA for me to contractually relieve GDCJFA and its staff from any and all liabilities, claims, demands, or causes of action, including but not limited to negligence and negligent acts, as described above. I certify that I am signing this form freely and voluntarily and that I understand that by signing this form I, as the parent/legal guardian of the minor indicated above, am giving up substantial rights.

I hereby give permission to GDCJFA to use any and all audio and visual recordings (including but not limited to photographs, audio recordings and videos) where my son/daughter/ward may appear for any and all reasons including, but not limited to, creating advertisements, booklets, informational pamphlets, and decorations.
